Before you begin, please read carefully:

This application is for childhood cancer or blood disorder survivors and patients who are currently in or have completed treatment.
Applicants must currently reside in Arizona or have received treatment in Arizona. Arizona residents may plan to attend school in or out of state.
Scholarship funds maybe applied toward tuition, books, and housing in accredited universities, community colleges, trade schools, or vocational programs.
Scholarship awards range from $500-$1,000 based on available funding and individual need.
1 letter of recommendation from a non-related individual is required. The letter must have been written within one year of the application date and must include the recommender's full contact information.
It is advised to prepare your essay responses in a separate document before beginning, so you can complete your submission in a single session.
Application deadline: May 10, 2026 at 11:59 PM MST. Incomplete or late submissions will not be reviewed.
